I just started the game.  Unlucky play, looked bad.  I think you're right and he was knocked out standing and so for the 2nd hit coming in he was basically a rag doll.  His neck would be limp and so any further action is just like spinning around a 20lb weight on the end of a string.

If they are right and he was moving his legs, that's the best sign he'll probably be ok eventually.
